Entry Information
1. Must
be a member in good standing with CHSRA to compete. Must show membership card at check-in.
2. Fill
in and total the amounts for each event in which you will be participating.
3. Complete the “Release
of Liability and Medical Authorization” and the “Grade & Conduct Verification”
sections of entry form.
4.
A current report card must be on file
with your membership secretary, Tammy Birrer.
5. Mail the completed
form along with a check payable to “CHSRA DISTRICT 9” to: Jeanne Reyes,

6. Entries
must be postmarked by Date on Entry Form. (USPS
Certificate of Mailing recommended.
No metered, registered, or certified mail accepted.)
7. Late entries will
be returned unopened. NO EXCEPTIONS
8. Only
CHSRA members and associate members allowed in all arenas and on horseback.
9. Entry
forms received by rodeo secretary which are incomplete will be charged a $15 fee, assessed trash duty, and must be
corrected before the draw.
